I, as well as many other people, are at a loss for what to do with our level 20 characters that have finished every mission and quest in the game. I found some "boredom relief" by trying to aquire every skill for my professions but actually aquiring the skill points to do this was daunting. Anyhoo. I have two suggestions on what could make the "end game" more interesting:
1. Create a "super quest". There should be one or two behemoth quests at the end once you've beaten the final mission. An example would be a huge quest that would have you collecting "lost tokens" or coins or what-have-you. Basically this would be a quest that would have you revisiting/exploring the majority of the world for possibly 100 of these "lost tokens" which would be dropped by either boss characters or some other character that only spawns when this quest is active (like, say, big assed dragons with a gold or silver aura around them). These characters would be strewn throughout Tyria both in missions and normal areas. This would promote exploration of the entire continent and give the anal "gotta get 'em all" players something interesting to do. The end reward for these huge quests would either be a shwack of skill points or possibly a collector that you could trade these 100 tokens for your choice of some amazing weapon or something. I'm not talking about some "uber" kill everything weapon but something with enhancements that you wouldn't normally find - just something better than your average run of the mill "good" weapon. Or, maybe more enticing, doing this quest would make it so that when you take on a different secondary profession you'd have access to all the skills you've unlocked from your other characters of that profession as well.
To give this idea a little credibility, you could compare this to games like Super Mario 64 where once you've played through the whole game there was still a reason to play some more so that you could unearth every single hidden star in the game.
2) The other idea hearkens back to Diablo 2. What did you do once you went through the whole game? Well, another difficulty level was unlocked so that you could go through the game again...but harder. The way that this could be achieved in Guild Wars is if, once you've finished the final mission, you were brought to an area possibly called "Chamber of Mirrors". In this area you could walk through one of these mirrors to enter an "alternate reality" of tyria. Basically, it would be Post Ascalon but everything would be much harder. This would allow you to do all the missions again but against worse odds (and maybe with moderately better loot drops).
Anyways, each of these ideas would clearly take time to develop and it's hard to say if Anet would think these additions would be worth it. I would just like "more to do" with my fully developed character but I don't find PvP in it's current state much fun right now.
