Why won't the first door open on the first level of the dungeon?

Make sure to kill Xien and pick up the dungeon key to open the lock. He doesn't appear at the spot where the key icon is on the mini map (well he does, but he disappears). You have to kill him in/near the last room.

I'm at the first door and it's closed. So I can't get to Xien. There's no lock at the first door.
Looks like I found another broken eye of the north dungeon.

Did you go in the door to the dungeon, or did you go in the door that starts the quest? Sounds like you're in the quest, which you access by going over the vine bridge. If that's the case, you open the door by talking to one of the golems in the front of the room and let him open it for you.

If it was truely broken then nobody would be able to do the Dungeon at all.
I have done all the Dungeons in NM and HM and so far none of em was bugged, so it's more like a random glitch then broken.

Is it the one where you have to get one of the Golems to open it for you?

You're thinking of the Oola's Lab quest (whatever it's called) that's part of the Azura story arch. The dungeon quest "Little Workshop of Horrors" is the Dungeon quest for Oola's Lab where the problem with the door comes in.

If you don't have the quest (Applies to other dungeons such as Ooze pit) then you are very limited to where you can go as there will be closed doors.

If you travel to Oola's Lab without a related quest, then you will not be able to open the first door.

In order to do the dungeon itself you need to talk to Blimm in Rata Sum to get the quest "Little Workshop of Horrors" additionally, you must not talk to Blimm outside of the dungeon as that is part of "Lab Space" quest.
