This is what seems to be happening,
Code = 015 is effecting RA (It's normally an Internal Server issue) what this means is that something within the hops isn't functioning correctly. In reality this is bound to happen every now and then.
Code = 013 This started happening about the same time the Zaishen Bounties, Zaishen Missions, etc changed over this like 015 is either a server issue or caused by an Update. The update isn't likely to be effecting it as nothing was done to the Guild Hall but Code = 013 is being reported mostly in Guild Halls.
Code = 040 This is a Code I've never seen before, it's not documented within the wiki so I can't give a lot of insight as to what it means, I'm assuming it's network related as well (just like 13 and 15). I only experienced this today when I was doing the Zaishen Bounty and our party mapped into the area with the boss we needed to kill.
The error codes 004, 040 and 013 are also common in heavily populated areas of the game.

So it is, haha. My assumptions were correct then it's a network related error code like all of the other ones. The thing I find to be the biggest problem is Random Arena's, when I'm in the outpost I have my normal 30ms - 65ms but as soon as I'm matched up my ping spikes to 15000+.
