I'm ready to begin my attempt at the Missions in Hard Mode and I'd like to know what other people think are the hardest for EACH chapter. I did Dragon's Lair in Proph and it was actually pretty easy, even the bonus.
So list what you think was the hardest in EACH Chapter, what made it hard and any possible tips for making it easier with h/h or if you think its impossible with h/h.

Prophecies: D'Alessio Seaboard. Rush to Dorian, you're screwed. Don't rush to Dorian, you're screwed. Tip: a smiter is your friend.
Factions: Raisu Palace. So. Many. Shiroken. *STAB*
NF: Jennur's Horde. Filter wouldn't allow me to tell how much I hate this mission. It was my last one, I got an alliance group, we finished it into a few tries... and never again I will touch this mission.

For Elona , Dzagonur Bastion . everyone run Cry of pain in their build and one person will call hexes so th person with the hex calls a target to hex and evryone else cry of pain on that target . dead. and you also need someone with a party wide speed boost..
Jennur's horde took us a while ..... just try to pull each group and have patience
Factions ,Eternal Grove We had 2 monks both LoD,2 MM , 2 warriors ,2 eles
1 on each side and 2 jugernauts on the left side , 1 on right. So just kill the monsters and when the other side isnt done killing them , have a party speeed boost and run to th other side to help kill then go back to your side
and they were all human players .
tyria , D Assalio seaboard , Dorian dies too fast, need a monk with alot of speed boost run to him and prot him then kill the monsters. and bring a smiter

Prophecies - Nolani Academy. Even with my monk the end was hard because of Rurik's Leeroying. A MM definitely helped and killing a few charr before Rurik moves after the cutscene also.
D'Alessio Seaboard is also annoying because of Dorian getting attacked early.
Dragon's Lair was easy compared to a lot of the other missions. Honestly, I found it a lot harder in normal mode. Why? Because for some reason Glint kept spamming her skills when I did it as my assassin. I had never seen that happen before and it took a while to get the upperhand.
Factions - Eternal Grove. I had a guild group doing this when my monk was going for the title, and it was horrible. We had so much difficulty defending the left side when the afflicted come. Of course this was when HM was new, but I still don't look forward to it.
Gyala Hatchery can be annoying, even if you do the backroute. It's kind of long, and if for some reason you lose a turtle to the large group that spawns at the start, have fun redoing and clearing all again.
Nightfall - Gate of Madness? I guess it would be that one since so many people have problems with it, but I really didn't find it all that hard since my assassin H/Hed it. He also H/Hed the other considerably hard missions in that campaign, like Dzagonur Bastion and Moddok Crevice. Guardian of Elona is the easiest to get out of the three imho.
Gate of Pain was kind of annoying, but if you can shut down the spiders quickly then it's a walk in the park. Just have to make sure you don't aggro too many tormented groups at the end.
D'Alessio Seaboard is also annoying because of Dorian getting attacked early.

I haven't done much with HM missions in Prophecies yet. Though any I have we completed in the first or second try. Even Aurora Glade was easy, beat it on the first try.

And Kus, I didn't find Rasisu Palace that hard at all. I had a guildie group and we rolled through it beating Masters time by about 2-3 minutes.

Cantha I would say Gyala Hatchery.
Tyria... hrm, pick a Kryta/Magnuum mission. They're all equally boring and dragged out, nothing really too difficult.
For Prophecies I suppose I will say Nolani was the hardest, due to Rurik. I did it on the first try with Heroes on my monk, although at one point near the end Koss and Rurik were the only two standing.
The one I actually had the hardest time with was Riverside Province. Had no trouble at all with D'Alessio Seaboard. Go figure.

Tyria: Dunes of Despair without using Necrotic Traversal.
Factions: Eternal Grove (though i finished it at once but i knew how to counter basically everything).
Elona: Dzagonur Bastion (even tho it is not that hard).
I don't know why people say that Jennur's Horde is so hard. But i found that one of the easiest missions to do in Elona.

Tyria: D'Alessio because of Dorian as people have said. Riverside Province was a pain in NM to rescue those villagers. In HM it's worse.
Factions: I actually had some trouble with Zen Daijun. But Eternal Grove was the hardest. I teamed up with a good warrior there and we were able to beat it after a few tries with heroes. I found Raisu Palace to be a breeze. Spreading around conditions helps finishing the mobs.
Elona: Suffers from incredibly easy to horribly difficult. D. Bastion took me and my guildies several attempts to finish. I h/h'd Jennur's Horde. This was a terrible experience. It takes a long time to do by yourself. I found Gate of Pain to be harder than Gate of Madness. But this was because I had an older computer and the lag from the first group would wipe me.
I got Legendary Guardian on my Ritualist before the first pve skills came out. So I'd image that a lot of HM missions are easier now with them. I know that Summon Spirits would have been extremely useful.

Tryia - The Wilds. Moss Scarabs, fast moving Centaur patrols, the necessity to pull almost every single group to not over aggro, and did I mention the ^&#$^ing Moss Scarabs.
Cantha - The Eternal Grove
Elona - haven't finished but Dzagonur is proving an obstacle.
And since you're asking about H/H, I'd say that Aurora Glade and Moddok Crevice are impossible without a buddy, but are relatively easy missions otherwise.

And since you're asking about H/H, I'd say that Aurora Glade and Moddok Crevice are impossible without a buddy, but are relatively easy missions otherwise.
Prophecies: Aurora Glade bonus if you try to tackle the Mantle head-on (you can't do it, just don't try), and/or if you're H/Hing it. D'Alessio Seaboard otherwise. The siege on Dorian starts sooner than you want it to, and it can be hard to recover.
Factions: Eternal Grove, hands-down. It will be one of the biggest adrenaline rushes you've ever experienced in Guild Wars, but it's tough as nails to last through the Afflicted sections. Tahnnakai Temple is bad, but bad in the "just plain annoying" way.
Nightfall: Dzagonur Bastion unless you're bringing 2 MMs, then it's easy. Ruins of Morah can be a nailbiter unless you can drop the patrolling Torment creatures fast enough.
