I suspect it works like Endure Pain. When Endure Pain ends, it can drop you below zero health if you only had the bonus health from Endure Pain. You will still have 1 hp, but the next time you are healed, take damage, or suffer negative regen, you will die. You have to wait for natural regeneration to increase your negative health past 1 before you can heal. Apologies if you already are familiar with Endure Pain and negative health.
You could test yourself fairly easy with 55 armor, a vamp weapon, enchants, Symbiosis, and Contemplation of Purity, or any other skill which removes all enchantments on you.
