...and found a chest:

(There's no way you could get to it. Somehow it spawned on a patch of unpassable terrain.)

Yes, chests can spawn in unreachable areas. This isn't a new thing.

i had that once , it spawned in the deep area of a lake , really far in it too

Yeah, on Ice Caves of Sorrow I've had chests spawn all the way out on those Siege boats near the end of the mission, waaay out into the middle of the lake, really odd.

Yeah, it's old... they spawn like this a lot. I've seen one spawn in a hole where afflicted come from lol... and I could open it but couldn't pick up the item
