Missing town in Realm of Torment

Hi, i didn't find any threads with this already posted, but i was wondering if anyone could help me find the town im missing in the green circle... and also if any knows if theres any towns in the middle in that small circle, oppsite of the vortex, that would help out alot.

if you could tell me a town and a direction to leave from that would be great!

East exit from the Gates of Torment town.

No-one knows about the south circle, but it's thought to be the potential site of the elite mission.

How do you get to the Nightfallen lands (NW town)?

There is a portal to the northwest, right near that rez shrine. Kinda hard to see unless you look towards the south when you get there.
