I saw this guy this morning in ToA, looks really weird though it looks more like a glitch then an actual helm.

It would be really scary if there really is a 15k knight helm, and that guy know where to get it to fit his platemail.

Theres always some foo screaming out photoshop when they see something that doesn't make a whole loe of sense.
If that was a the case I bet all those screenshots of monks with invisible tats are also photoshop even though those are glitches also.

Nope its a known glitch [I hate it when people jump to "PHOTO-SHOP ZOMG" errors"]
All you have to do is minimize Guildwars and during that time if someone was wearing a helm and then changes it. When you maximize guildwars, your local memory thinks the new helm is the old helm [but the serverside memory changes the model] so your computer locally keeps the skin of the old helm. In some occasions, this works for armor too.
