Many religions are not as grandious and "public" as your more modern-day organised ones (Christianity, Judaism, etc.)
Most faiths will have some specific spot that they associate more with their religion than any other spot, that's true. This spot doesn't need to be a structure though. It could be a campfire, a tree, some marks on some object, a pond, etc.
Other times it could be the religon specifically prohibits (or disapproves of) any materialistic significance. So in that case, you wouldn't find a "place of worship" at all.
If you look at the various places in Prophecies, you can point out specific places of worship (sacraficing circle in the wilds, temple of the ages, inside volcano towards the end of prophecies etc.)

There's only 6 gods: Melandru, Dwayna, Balthazar, Lyssa, Grenth and the God of Secrets/Goddess of Truth (depending on whether you look at the timeline before or after the events in Nightfall). They don't have a "religion" of sorts, but all the professions that the player can choose in the game all have their deities among these gods and there are religious orders such as the Lyssa worshippers in Vabbi that clearly stand out.

Wasn't Mhenlo born in the Serenity Temple? I'm not sure if that temple was a place of worship for the Tyrian/Elonian/Canthan gods, or if they just worshiped at the various shrines found around (e.g. the shrine of Balth close to the breach, Melandru in Regent Valley)...
These seem like, at the very least, monuments to some sort of polytheistic religion. If the sixth god was defeated/killed/banished (I haven't completed NF yet), all trace of their existence in many area may have been eliminated as well.
But...i haven't come close to finishing factions yet (haven't even made SS Gen. yet.) so there may be some details I'm missing.

Well It might be safe to assume that the towns people have small shrines in their homes. Most polytheistic religions require grandoise temples but maybe their in the other parts of town you never get to see...

Temple of the Ages, Serenity Temple, Ashford Abbey, and the bonus for D'Alessio Seaboard (I think) all give some insight into the worship habits of the people. To what extent is unknown, since all of these areas are either already destroyed, get destroyed, or are miniscule. I doubt people gathered for mass every Sunday in the Temple of Ages, but I think that people would commune with the gods when they needed to.
The gods don't strike me as the "worship-me" religion. They don't require worship, merely recognition of their domains.

There is no Goddess of Truth, her real designation is Goddess of Secrets and her avatar is called Seer of Truth. Thus the Goddess replaces the old maddened God. As history is written by the victors, the gods removed Abaddon from history writings. The GW panteon has great similarities to the greek panteon, in which there wasnt really a fixed number of gods. Simpy creating a new god would fill an old spot or create a new one in the panteon

Though there are locations for worshipping, not all religions / cults have churches / temples.
Some cults would just worship an idol depicting an image of their God(s).

Although not all religions dont have temples and shrines something tells me the Ascalonians are far too enmeshed in faith to not have a massive temple to atleast all the gods at once. Many major cities in all polytheistic religions boasted large temples. Rome, Athens, Sparta, Delphi each one had massive temples to their gods and a larger one for their patron. Many eastern religions like Bhuddism have truely massive complexes. And usually their god/idol is in a special public place (atleast for the members of said group) making it a temple.

Well, Serenity Temple is, I believe, described as being the Ascalonian equivalent to the Temple of Ages in Kryta. It is also described as being ruined after the Searing (and we don't see it before the Searing).
While the only statues present are to Melandru and Dwayna when we see it, it could be that shrines to other gods were there, but were destroyed - or it could be that all the statues were destroyed, but that Dwayna and Melandru happened to be the first to be reconstructed (for a certain value of "reconstructed" when it comes to statues of Dwayna when you don't have favour...)

Also recall that Vabbians openly worship Lyssa, in fact many believe it was shaped according to her wishes (various conversation with NPC's), and the floating shrine above the Mirror of Lyss is an altar to Lyssa herself.

What proffession you are matters who you worship, and you don't bow down, your worshipping them is fighting as they're disciple and using their favored combat style and LEARNING from them.

Ged that is true like in handbook says proffesions belive in what different gods say which is why wars and eles worship balthzar and necros worship grenth and idiots worship abbadon LOL
