It seems if you transfer your character from tyria to cantha, you miss on the Cantha starter island.
This makes it effectively impossible for old characters to get 100% on both continents.
Not sure if this is intended or an oversight, or if that area is accesible later in one way or another.

You can take a boat to pick up the Monastary area of Cantha... so it is not impossible...
Not sure where the boat or the person that takes you to the monastary, though I know it is possible. (I will have to ask a guildie)

I believe that is pre-order only.

So did we figure out that arenas do actually count?

Arenas do count, but there's enough map outside of arenas to reach 100% in both campaigns.

You can get to Shing Jea if you're a foreigner, and you don't need to have pre-ordered it, you just take a boat from the port by the Marketplace. However, there is a very tiny tutorial section south of the Monastery that accounts for like 0.1%, and it is skippable even by native Canthans. And you can't return to it. That is really bugging me at the moment, as it is an incredibly simple tutorial, so i skipped it with all but one of my Canthans

Guys, I was in this same position last week at 98%.. here are some simple facts:
pre-searing does NOT count
arenas do count BUT, you can get 100% without entering any of them ( My map is proof of this, as I didnt try pvp till the arenas were moved to the battle isles)
There are a couple of really good examples of 100% maps (closeup's) posted which will help you, this is how i got my 100% in the end
So it is achievable, albeit really difficult on the last 2%
