Default The Second Vision not showing up?

I just finished seeing the Final vision on my monk, and I noticed (spoilers)

it goes first > third > final. I seem to be missing the second vision. One of my friends is having the same problem as well. We both beat the Norn side during the preview...do you think this has anything to do with it?

I think it means you didn't look into the pool when you were supposed to. I'm in an even worse situation - I only have the 1st and the last visions ;/

I don't think beating it during the preview matters, because I didn't and I have the exact problem you're describing.
