Usually a fair mix between keypad and mouse. It mostly depends on how bunched up people are if there's going to be miss-clicks. For running, I'll move my keyboard all the way left on my desk and move with my left hand on the keypad and camera/skill with my mouse.
I want all my skills to be at my finger-tips and I prefer to move with keys. The problem is that camera and dismissing enchants really requires the mouse.
I like the theory of ManaCraft's layout. It might be too funky for me to get used to, but I'm definately going to give it a try. (we'll see how smart my left pinky is)

I think it should be a standard that new player should put their chat window higher than default so they don't miss or forgot to read messages

Noob Question: How do you rotate stuff so that the health and energy bar is vertical? Also with the skill bar Patrograd, how do you make it so you have 4 skills on 1 row, 4 skills on a second row.

just click and hold the size of the box and shrink if from right to left then make it big from top to bottom.

Interesting arrangement. It provides a lot of free space, but I'm not sure I could get used to having everything in the upper corner like that. I like having the center area clear with most of the feedback in the lower center.
I'm not very good with using key commands, though... I use the mouse a lot. I'm constantly moving my view around as I'm fighting/healing to reposition. I prefer to right mouse-drag + strafe to turn. In fact, I changed my A and D keys from Turn to Previous/Next Target. I keep my left fingers over the 1,2,3 keys in combat... reach over with index finger to hit 4. I can act pretty fast that way and keep the most used skills there. Rez, and maintained enchantments go in 5-8, where they don't have to be used that often.
Should I be practicing using only keyboard movement to become more efficient and "fast acting?" I've set skills 5-8 as 7,8,9,+ on the num pad, with num 0 as an alternate for targeting Nearest (where I can "slap" it with my right thumb), but it's not something I can naturally use. It works well. I can position with the mouse, then move my right hand to the num pad for targeting and skills, but I seem to have a hard time making it intuitive. As a monk, trying to target party members with keys would be slow for me, I'm afraid. It's easier to click on them with the mouse.
Anyone have any comments on correcting what may be bad habits for "fast acting?" I'm always looking for ways to improve my game.

I go a-s-d-f for party members 1-4 and q-w-e-r for 5-8. That's the typists left hand keys and one row up.
I use the arrow keys to move and strafe is one out. I had to rearrange a few keys. Basically, anything that's battle related is on my left hand on the keyboard and anything not is on the right with the directions as shown.
The only time I ever click anything is if I take Veil and want to click off that ench. I leave my cursor right there. I lengthen the health bars a bit but kept them pretty much standard.

Number Bar 1-8: Skills 1-8
Num Pad 1-8: Party members 1-8
I usually have the best healing skills (i.e. Word of Healing, Infuse Health, etc) as my first few skills on the bar. My res signet is ALWAYS at the end. I have my skill bar and such on one side and my party window on the other along with my weapon set (so I can double check if I'm using my high-energy focus, which I don't want to if I'm not getting targetted by a mezzy).
I'm too hyperactive to keep my eyes in one spot, so I made use of this by putting everything everywhere, with the battle in between so as I check if skills are disabled I can watch what's going on.
I haven't changed the UI since I set it like this, despite the fact I haven't played monk in probably a month
