I'm not saying this setup is necessarily the best but I use 12345tgh to select party members and qwerasdf for skills 1-8. Click to move I find alot easier than keyboard movement. The only downside of click to move is when you're in a crowded area because you might click on something by accident, but then your team is bad and you're gonna get killed quickly regardless...
If you have good positioning click to move is fine
Edit: I also use tab to cancel skills since it's quicker to cancel stuff than the escape key. Then I swap between targets by using C for closest(same), X for next target and Z for previous - this allows me to cycle through targets very quickly and in addition to seeing the casting animations, I can usually see what's coming and get a little more time to react.

Make sure that all of your skills 1-8 are easily accessible to one hand. (I use 1-4 [piano hands make reaching 4 very easy, I also don't use my left hand index finger], q, e, z, x) If you can also make party member selection easy to map to that same hand as well do that. Though the more you watch the field, the less benefit you will receive to mapping party members to keys.
If you really have a lot of keys left over (which depending upon your hands/keyboard/mouse setup you might) creating macros of infuse#1, so forth is also beneficial (or whatever emergency skill you us, spirit bond/rc/etc). Though without some non-usual setup, you won't have the type of set up that will allow that many keys to be free.
The point is to require no mouse movement, and as few keystrokes as possible before you execute your spells.

I use close to default controls. Right hand has mouse for movement and targeting party members as well as vent on a button. Left hand controls skills, weapons, spacebar (the most important button).
I remapped skills 7,8 to R and T for ease of reaching. I also moved my cancel button to tilde (~), again to make it easier to reach.
At this point, though I just tell everyone to play something comfortable. Default really isn't that bad a setup so long as you can reach everything you use on a regular basis. Most peoples' control schemes reflect what kind of games they played immediately before Guild Wars. People that came from RTS tend to have all of their controls mapped to their keyboard hand with the mouse reserved for movement and targeting (basically, like mine, but I'm terrible at RTS). People who played a lot of FPS's tend to move with WASD and have more clicking involved in actually doing stuff.
It's also worth noting that most, if not all, of the competent monks I know (knew) target most of the time via clicking character models. The vast majority of the people I know (knew) who are competent at kiting click to move.
And that click-to-move is more efficient than keyboard-to-move in a very real way (in two very real ways, actually), but if it feels awkward, you should probably just keyboard it. The number of people on obsmode who move well can pretty much be counted on one hand anymore; it's no longer a required part of the game to play at a relatively high level.
