I only bother topping people off with heals if it is incredibly efficient to do so, via signets, Heal Party, or HoTs. Your job isn't to keep people at full health, it's to keep the party from wiping. If someone's health is low but they aren't taking any damage they're a low healing priority. If a softie is taking a pounding and refuses to kite the best thing for the team is to stop the damage by letting them die.
If someone has taken some damage and tells me to heal them, I start picking out enemies and order the noob to attack them. If they click on their health bars, spam targets. Granted I'm a good enough monk that I can back it up, but I do not react kindly to being treated like an idiot.

I work on keeping thier hp topped up a much as possible and use Orisons for the most part with enough points in Devine favour and healing you won't have to have to many problems.
lately since a past update ive been keeping vigorous spirit on the warriors and rangers if they are using barrage. at max healing its a 21 point heal for them each attack for 30 seconds, cost 5e. ive also carry Dwayna kiss, another 5e heal that gives extra bonus for enchantements and hexes on the target your healing, ill use Dwayna Kiss on those that i put vigorous spirit on at the start of a battle because in most cases if their health has dropped to 50% one shot of dwayna's kiss and they are back to near full health for 5e, in some cases dwayans kiss for me is my most powerfull heal for such little energy. add in word of healing and orison of healing and i rarely have energy problems. usually if im healing ill have 16 in healing prayers and 13 in DF if im carrying full heal skill bar. other times ill drop DF to 10 and up protection to 10 and carry mend ailment. a nice 5e condition remover that will also heal if ally has multiple conditions.
as for what i do if someone dies while im monking, if its my fault i have no problem saying sorry. if its the other persons fault do to over agro or not listening, or getting out of my heal range, then they get a warning first. if they still dont listen then usually one death will change their additude because ill just concentrate all my heals on those i can heal without getting my self into trouble by taking unnecessary damage.

2. Blame the dead player.
3. Blame George Bush.
4. Blame Anet.
Under no circumstance are you to ever, EVER take the blame on yourself.

Seriously though. Monks are in high demand. Be honest if you make a mistake, and accept responsibility-just a simple death (not team wipe) will usually be shrugged off by most players. If someone dies through their own fault, and they try to verbally abuse you, tell 'em where to put it. If that fails, departure is a viable option. I'm not one for quitting generally, but I have no intention of staying and getting reamed for half an hour due to someone else's fault.
Hate being branded a quitter since im nearly the last one to leave any groups that im in when the missions goes horribly wrong due to players not knowing what to do to complete it.
I try to offer tips in Abbadons Mouth like dont stay under the Nukes till you die,not just 1 player but most are just happy to stay under the nukes and then wonder why they are dead.
So when one person spammed that i was a quitter.
Gave up the idea of being a healer for groups and henched mission instead.
yup with the henchies completed mission and bonus .Popped back to check who were still in need to complete only too see many ex groupies spamming GLF monk blah blah blah.Thank goodness for Henchies
What pisses me off is when someone dies, and then they spam "Rez" as if the group couldn't possibly go on without them, when we are still heavily engaged with the enemy. Take Vizunah Square, some guy get's spiked by 3 ele's, and Mhenlo, as usual, is bravely trying his best to tank and is getting pounded into mush. Lets see, do you spend the 6-8 second's it takes to rez someone, or do you keep Mhenlo's stupid ass alive. (no matter how much I like seeing Mhenlo die)
I once had my level 20 monk from Tyria heal for a group of level 11-13's in the second factions mission, Zen Daijun. The group was made up of 2 assasins, a ranger, a ritualist, and a minion master. Afflicted explosions would take away half of the life from each of the assassins, the ritualist would set up his spirits to in the middle of the battles, and the minion master died attacking a far away group and quit. The group members fought battles against multiple groups while standing in vile miasma for added degen. I swear that most of them were wearing starter's armor.
The entire group wiped at the two bosses near the end, so I was left as the only one alive and I healed the level 15 shock elementalist NPC and Master Togo. I pretty much wanded those two bosses to death. Resurrected the dead ones after battle since I know my priorities. Surprisingly, ridiculous scenarios like this happen very often and there's nothing anyone can do about it.
A common urge that some monks get when someone dies, is to begin to resurrect that person immediately. I don't want to say you should never do it, but you need to be aware of whats going to happen in the next 6-8 seconds if your trying to rez someone. As the monk, you have to be calm under the pressure and control the situation... if they get one, make sure they don't get the next. Maintaining composure is the most important thing to do, because alot of times somebody has to do something spectacular to save a wipe. If you're the healer, then that person is most likely going to be you. Being nervous under pressure situations is only setting yourself up for failure, despite whatever talents you might have.
Eventually, you get tested to your limits to points where you find yourself keeping the last person alive, while having to Rebirth the other monk while both of you are under attack. Fun stuff.
I always blame myself, and get stressed out about it.
This is why when I'm monking I'm more likely to snap at people and get bossy than when I'm playing other classes.
Me too, my biggest pet peeve (okay, that was not a B/P pun ) is people who don't unequip vamp weapons when not in battle. They keep them on even after I tell them not to equip them when they're not fighting. That just gets me for whatever reason. I do also find myself more irritable when I'm monking
I'll try not to repeat anything on here, cause it's all great advice, but i'll try to add a couple things.
Remember that if there's another monk coordinate who's healing who.
Be careful of necros, cause sometimes their health needs to be low(talk to them before hand, cause if they can they'll often bring blood ritual for drawn out fights)
Only thing i can really say is be polite and hope they are too. If they aren't, ditch at the nearest town, Use "Comfort Monk" to cheer up, and keep on truckin'
In my personal oppinion, to be an effective monk: You need to be an asshole.
Personal Rules I go by:
(1) NEVER Resurect during a fight. No matter how much that player is bitching at you, chances are someone else will die while you are resing. Keep healing what is alive and put your energy to good use. Keep group DP to a minimum that way. If ANYONE should Rez during a battle, it should be a non-healer.
(2) If he likes taking damage, let him take damage. Some proffessions with low armor will run out and start taking loads of damage. Usually they dont bother to heal themselves, OR do the smart thing and run away. To the kind monk, this proves a pain in the ass as this QUICKLY drains monk energy while you constantly heal. In my oppinion, if a player is drawing too much damage, let him die so that your other teamates arnt affected by loss of energy.
(3) OMG Noob monk heal me! Usually this happens if someone runs off without warning to go pull a mob. If you decide to do this without warning me, then your on your own. Let the designated puller pull, or just dont run off on your own. Only healing I will be doing is scraping your sorry corpse off the ground with a pooper scooper.
Individualy people might hate me by my moral. However GW is a TEAM based game. Im mainly concerned about the good of the team instead of individual players.

Wait, dont heal for about a 1/10 of the bar, you might aswell wait till low, and rack on a dwaynas kiss / infuse health.... then touch yourself.
NEVER , no really NEVER res in battle, theres only been one exception, really one, last day dawns, i was a lvl 19 monk - 6 man team , left with me and an ele, he tanked, i ressed, he died, i still ressed , i survived, on about 50 hp which isnt good, what i am saying is - i was lucky, i was asking for death, ressing in battle is a no no, if the ele wasnt dead i'd of been prawned
