Hmm, perhaps one has to watch what they say on GW forums.
While farming towards my LB6 last night I swear someone was manipulating the environment behind the scenes - testing the environment vs. my build?
Maybe I was just paranoid and tired, but target group behavior seemed to change, and the couple of bosses in the Citadel at DoS started really kicking my butt.
I had visions of some master puppeteer programmer behind the scenes cackling away - "BWAHHAHAHA" - as he manipulated the game environment for his amusement, and to ensure I earned those LB points as painfully as possible.
On one hand, game developers probably do continually test and tweak the play level within the game. Though I don’t imagine they have an actual dial that they can crank up to butt-fry during game play.
On the other hand, it could be that any build alterations I made either were backfiring or changing target behavior in some way, and I just wasn’t compensating. And perhaps I had gotten cocky and was playing a little too loose and reckless.
More trials tonight…

Minion master (Fiend, Horror, BoTM, Order of Undeath, Signet of Lost Souls, Mantra of Inscriptions, Leech Signet, Rez Sig)
Searing Flames (SF, GG, Glyph of Lesser Energy, Liquid Flame, Leech Signet, Fire Attunement, Aura of Restoration, Rez Sig)
Searing Flames (same as above)
All 3 characters have both a mainhand and an offhand that is 20/20 HSR and 20/20 HCR for their most important attribute (you can just craft these from the Vasburg Armory, no need to get greens) and stuff dies FAST.

For my ranger i always take at least one of the rangers and often i'll take both with me. the boost of being able to cause lots of conditions(burning, bleeding, poison, lots of interupts, and pets can really make my hench party pretty hard to beat(having 3 extra level 20s with the pets is really useful sometimes)
Olias as a MM is also pretty common for the same reason as the pets, having up to 8 minions can really take away the challenge to alot of confrontations. That being said sometimes minions can get in the way of being sneaky, so i trade him off for either melonni as extra melee pressure or one of the monks as a protection monk.
Not a huge fan of Koss and haven't figured out a way to make Zhed work well enough to bother to bring instead of Herta or Cynn(i need more elementalist skills i think, haven't really played an ele myself much)

I'm playing a ranger and an elementalist, on both I use Jin (barrage or BHA, distracting shot, savage shot, favorable winds, throw dirt and three others that vary) Tahlkora or Dunkoro (Aegis, Mend Conditions, Orision of Healing, Healing Whisper, Healing Boon, Power Drain, Leech Signet, Resurrection Chant) almost all the time.
The third varies heavily on requirements, and expectations. General Morghan in a support chant build is very good at keeping everyone healthy, so I can choose more damage dealing henchies. On speed runs (as a requirement or just for fun) I like Zhed as a second elementalist. On slow and steady runs, I play an MM hero.

When I really don't want to mess around, I take three Searing Flames nukers (one primary) and an MM.
Mirror of Disenchantment is a bit annoying, but I'll probably stick with this approach even so for a while. I haven't really gotten to places in the game where a lot of enemies don't leave corpses.
The rest of the party includes two henchie monks, one henchie tank, and a variable fourth henchie.

Myself being healing monk + Tahlkora for prot, Olias as mm+battery and magrid stashed with interrupts. It's a very good foundation to build a team on.

Often Jin+Margrid+Zenmai and their pets.
Set all to barrage, demolish enemies, repeat. Fun with 4 pets out there tanking. Tweak bars as needed, substitute Broadhead arrow for example for versus certain bosses. Sure, it's a bit simplistic, but it works well.

Everyone who uses Zhed + Souske + Olias has the same idea I do. I find that two Searing Flames plus a Minion Master cranks out the most damage that I've been able to milk from heroes. Typically the problem from using hero/hench parties is that the henchmen don't do enough damage. That is compensated for by using extremely high damage oriented hero builds. I shy away from using the monk heroes because the hench monks are competent enough that they can handle most situations just fine.

As I think I mentioned before, that's the max effectiveness build for me in most zones. Counting my primary character:
3 Searing Flames nukers (with interrupts on the two heroes)
1 MM (that build keeps changing)
2 henchie monks
1 henchie tank
1 henchie variable
Bad stuff rarely happens to us, unless I'm being very careless ...
And I don't have the heroes' armor maxed out with the best runes or anything.
