Undead vs. Smite?
I know holy dmg from smite skills does double to undead but when I was going through the game I noticed sometimes it would be quadruple damage.
Possible bug? Or is there something about damage calculation I'm missing?

It could be that the undead was attacking and the extra damage could seem like quadruple.
Also it could be that some undeads have armor against smite.

holy element smiting damage ignore armor... so no matter what the armor the undead got, same deal...
there are things like frenzy and so on that double the damage they take... although I don't remember any undead that has it...
Right now, I can only conclude it that you calculate it wrong... as you provided as good as no info for us to really do anything.

I believe i've seen some undead around that did use frenzy (Might've been those bull-like executioner guys) meaning another double damage right there, so that's probably the real best bet I could give. I don't know if holy damage really penetrates 100% armor seeing as how that would make every damage done from anybody be equivilent to the damage a hell hound takes, which can be anything from 100 - 500.

No undead armor defends against holy damage but some take EXTRA holy damage. Make a PvP necro and look at their bottom armour 'Takes extra damage from Holy Element'.
I've seen this too, I suppose that those undead just had less resistant armor and at times it seems to happen at random. Random armor types? i don't know.

no the holy smite go over the armor... and the necro set does not make you an undead, it is just built in to take more damage. The one doing more damage is the element. The funny thing is... once it ignore armor, resistance does not come into play... that means obsidian flame will do the same damage on a ranger with +30 elemental resistance as to 0 elemental resistance. So, anything that does weird thing... at most is probably some built in feature. (Also, I don't remember my smite doing weird damage unless block by shielding hands and such in PvE.)
and it is not armor penetration... it is ignore armor. Or else, perhaps you might want to start explaining that how smite do exact same number damage as it says on the description on other creatures.

Holy damage treats all targets as having 60 armor, at 60AL you do the same damage as on skill description. Less than 60 & you do more damage on the target, more than 60AL & you do less damage than skill description.
(If there is somehow something that gives 100% armor penetration you will do ALOT more damage than what description says).
Armor & damage calc you can play around with it to see what happens.
Edit: game mechanics page explains this also

They have a natural -holy AL, which accounts for the standard double damage, but also on occasion use frenzy, or at least some of them do, and take triple instead.
Don't believe I've ever seen them take quadruple.
32 damage symbol of wrath -> 64 on undead -> 96 on undead using frenzy
The Hellhounds definitely use something, because if you watch a sustained spell like Symbol of Wrath or Balthazar's Aura, it'll increase suddenly for no apparent reason temporarily. Although, once it increases, it's likely they're dead before it's over.
Hit a large group, and you'll see some taking the standard double, and some taking triple.

Sorry for the bump, I completely forgot about this thread.
It happened in Riverside and in Sanctum Cay.
Since posting I've come to only 2 conclusions. Hellhounds use frenzy, or it had to do with effects from the Scepter of Orr that you carry in those missions.
I'd cast Bane Signet which would normally do 30 damage and it would come out as 120. Likewise Symbol of Wrath would come up quadruple as well, though I don't remember the actual numbers.
Edit: It mainly happened on hellhounds but it did happen on some executioners and skeleton archers.
