Conditions will not stack.
That is to say, if you poison the target with two different skills, he's still just poisoned as normal. "On fire" is a condition as well, so a person can not take on fire damage from multiple sources.
Conjure Phantasm, however, is a hex, not a poison. So you CAN apply conjure phantasm at the same time as poison and the effect will be increased.
It's worth noting that no matter how many hexes and conditions you apply to the target, the absolute minimum for health regeneration is -10

I found that out today. No kindle with ignite....shame. You can however have a kindled hunters shot

Conjure Phantasm is a hex, and there is no other spell which inflicts that specific hex.
Therefore, the ONLY thing that won't stack with conjure phantasm... is itself (if you an another mesmer both cast it, it won't do any good).
It can be used with any other skill though

One exception...a Mesmer skill called Echo...you press echo and if you cast a spell within twenty seconds Echo becomes that skill. So you can cast out a skill TWICE and stack them

You can only use one type at a time.
I.e for a ranger, you can use a stance, tiger fury (33% faster attack), with Favourable winds (arrows 2x as fast arrows), with ignite (fire damage) and hunters shot.
Each are different types and will stack.
I am curious though as to how many Hex/enchantments you can have going on any on creature at any one time, as you cannot tell by looking at NPC health bar. Enchantments could be worked out in a player group though.

You can't stack hex or enchantments. When you cast the same hex on the same creature twice, you just renew the hex timer...you dont inflict/stack the hex effects twice.

I didn't mean that, I ment different types of hex's,
I.e A group with a cursing Nerco and Mesmer both hexing one opponent.
Do the hex's from the different class's stack / cancel each other out?

Ok.
As long as they are different spells altogether, then they'll stack on top of each other.
I remember playing my monk and got stacked with more than 9 hexxed/condition...very nasty indeed.

let's say your Phantom Pain actually causes Health degen of 3. Does this mean that target foe will actually have a total health degen of 8 (5+3) ?

Actually I made additional searches with other words and it would appear that 2 different skills/spells causing health degeneration will add/stack up but only to a maximum of -10. Same goes for health regeneration.
If you cast the same spell/skill twice on the same foe, it just renews the time counter and does not add the effect together

I've been in PvP and had Poison, Burning, Crippled, deep wound, bleeding, empathy, and pacifism on me all at one time before, I hope that helps a lil bit.

You can have different hex and conditions applied and stacked but not the same of each, thats about it. For R/Me good combo is poison + conjure phantasm giving 9 arrows of degen or incenediary arrows + conjure phantasm giving a max of 10 arrows of degen, nice effect :P
Also ranger preperations dont stack, so you can't have kindle arrows + ignite or kindle + apply poison, if you could then it would really be nice especially with marksman wager + kindle + ignite .... but anyway enough of me dreaming, you can however stack enchantments + preperations, good combo for rangesr is conjure fire + kindle arrows. Add in a ritual or two increase the dmg even more.
Note natural rituals do stack with each other.

I don't know if that's the same sort of stacking your thinking of(I don't know much about conditions, sorry, I like Elementalists and death magic :P)

Have been using Kindle arrows, poison arrow with ranger as its cheap and spammable.
I also always has savage shot equiped due to effectiveness againt casters.
Poison arrow, lasts upto 15 sec's and is applied individually to arrows so effects stacks nicely. Not got marksmans wager yet, sounds a good skill, but with high expertise only usefull if fighting masses of enemies and in them cases barrage is also very effective, although it doesn't stack with preperations.

Well you can marksman wager before ascension in the crystal desert while doing those 3 missions, normally when you fight a boss monster whose name starts with Custodian, which is always a ranger.

Not true. I am a Mesmer. I have a skill called Echo and a skill called Chaos Storm. I hit Echo then Chaos, Chaos storm starts and Echo then becomes Chaos storm pulling off -10 health per second. I then cast ANOTHERChaos storm wich was Echo pulling off another -10 per second. It is now knocking off -20 health per second until the Chaos storm ends...

Chaos storm isn't true health degen. It causes damage on the tick (aka each second) for a certain amount. True health degen is like poison or bleeding constantly drain health (and are shown as arrows in your health bar, but are invisible on your opponent).
