Depends on the Char imho. On a Warrior i wouldn't want any -hp, but on my Ele for example i don't care that much, if i get hit, wich shouldn't happen, i die pretty fast anyway.

I don't think you can apply a rune to an armor pice that already has one in it. But no problem, you can just more than 1 piece. Like 3 different helms, first with minor rune, another one with a superior and perhaps a third with a rune for another attribute, so you can change your equipment, when you change skills.

Applying a rune to a runed armor piece replaces the old rune with the new one. You're prompted to confirm this so it doesn't happen by accident.
As for which runes to use, one superior for a main attribute plus minors for supporting attributes is a good rule. Sometimes you don't want any superior attribute runes (some PvE Warrior builds and PvP Monk builds), and rarely you'll want multiple Superiors.
Majors have no place in competitive play. The only time you'll ever want to use one is because you simply do not have the Superior available. Major Fire or Major Axe is a fine rune to use while looking for the upgrade, but once you have it ditch that Major and never look back.
