Survivor Primeval Leggins of Clarity
- Health +10, Blind/Weakness -20%
Survivor Primeval Boots Absorption
- Health +5, Physical Dmg -3
Forgotten Hammer
- Dmg 19-35, dmg +15% (above 50%), Armor penetration +20% (chance 20%), Health +30

You're pretty close to the standard Urf Shakur build. All in all it really only has one huge problem - no IAS on the slowest weapon the warrior has. Flail is standard on a hammer bar since the enemies won't be able to move away from you, negating the only downside it has. I would pitch Rebirth for it - you should never be the last one alive and trying to Rebirth. Never.
There's also really no need for Save Yourselves IMO, since the best part of Earth Shaker is keeping an entire mob (or at least the dangerous parts of it) continuously knock-locked - it also pulls adrenaline from your Earth Shaker, and you basically want to be able to use it instantly when your target stands up. Crude Swing is a good replacement since it's another AoE attack, and you want to hit as many enemies as possible to get your adrenaline back up. If you do bring Crude Swing, you can find a mob with four targets (with FGJ up) and constantly keep them knock-locked (ES, Whirlwind, ES, Crude, ES, Whirlwind, etc.)
All in all it looks like you've got the basic knowledge of playing a hammer down, looks like the biggest problem you have is your armor choice

Zelgadissan has summed up basically everything you'll need to know, although I prefer to use a Furious (10% Chance of Double Adrenaline Gain) Hammer of Fortitude as Sundering is nice for the extra damage, but fast kills aren't as needed if nothing can move ^.^

I'd put crude swing over crushing blow, single target deep wound is useless in a mob situation and it helps to charge ES much much faster.
Rebirth is good, enduring harmony would be a good alternative so you get a 30 second FGJ.

You should have flail in there, drop rebirth for it. Crude swing is also nice allowing you to regain adrenaline quickly for another Earthshacker. You could probably drop your summon to fit that in there. SY can both be usefull and useless in this bar, try it with and without and see what you like more.

ECharge-FGJ!-Crude-Flail-ES-Whirlwind-2 optional
SY! is very good, but ideally you'd want another player handling this
Summon Djinn is meh
Enduring Harmony... 10 extra seconds of FGJ! is worth it imo (esp if you decide to take SY!)
Rush for those rare occurrences of Flail-lock
+1 for Furious hammer
and Absorption runes suck, gogo Vitae

+1 for Enduring Harmony and basic ES+crude/whirlwind chain. If yeti did actually hit on bonus for near targets ( same +20 ) would be awesome but if not , mokele smash/counter attack/crushing do the trick.

Not bad IMO, you just need an IAS, drop skill 5 or 8 for it. You might also want to use crude swing or yeti smash in place of skill 2.
