To get the 'extra' +3 you just add another rune to a different piece of armor.
They don't stack, but the health reduction does.
The rest of your health is lowered with a -50HP Grim Cesta from an early Prophecies quest called Cities of Ascalon.
600HP is easily enough reached with various runes and fortitude mods.
(However, 600 monks don't need 600HP. Although there's no reason not to have it.)

you have to have an offhand that reduces you HP by 50, for the 55, the +3+3 is TWO runes of the same kind (only gives +3 but is REQUIRED to lower your HP) example... helm (smiting with sup smite rune) chest with sup smite rune, gloves with sup heal rune, pants with sup divine rune, boots with sup prot rune, brings your HP to 105, with the -50 offhand, *attainable from a quest*; you get, 55 hp, with protective spirit,, you take 10% or 5 damage
for 600, you actually have a buff from your smiter, that gives +100 odd hp, so essentially.. you get as close as you can, they do the rest, just dont die

55 = a sup rune on each piece of armor, + the -50hp cesta from Cities of Ascalon quest in post searing:
480 -75 -75 -75 -75 -75 -50 = 55.
As you are using all these sup runes, ur attributes of primary proffesion will have +3 so the attributes u posted above would be the number + 3 for the sup rune. The +3 +3 +1 smiting would be a typo, or through the use of consumables. Candy Corn, Golden Eggs, Lunar Fortunes, Grail of Might etc.
A 55 survives through the use of [Protective Spirit] which reduces all damage to 5 since you only have 55 hp, and either through further reducing the damage: [Shield of Absorption] and [Shielding Hands], or through outhealing it through [Mending] and [Healing Breeze]. [Balthazars Spirit] + [Essence Bond] can provide tonnes of energy for you.
A 600 monk doesnt actually have 600 hp, however, its a tanker/farmer monk that uses alot of HP and survives by reducing damage to a decent level through [Protective Spirit] and outheals it through [Spirit Bond]. [Shield of Absorption] means that when [Spirit Bond] runs out, the damage you take is minimal and u dont get insta killed by large mobs.
600 is often run with a smiter using [Holy Wrath] and [Retribution] to kill the mobs quickly, efficiently and safely, whilst the 600 survives under Prot, SoA and [Spell Breaker].

just to spare any confusion cause op seems to be really... yeah... 600 monks don't need exactly 600 hp, it just has to be either 600 or higher, if it's lower it won't work.
cause you have prot spirit + spirit bond on you with lowest possible al armor, so you have to be getting hit for over 60 for spirit bond to trigger under normal circumstances (if you're feeling the need to jump down my throat about faulty game mechanics and order of events, save it, don't confuse the guy). so yeah 600+.

I believe this is incorrect. Until the buff to RoJ (build OP mentions is likely pre RoJ) smiting prayers superior runes were usually the cheapest monk superior rune. Since there are only 4 monk attributes and you need 5 superior runes (+cesta) to drop your hp to 55 you will need to double up on one. Since this gives no additional benefit it would make sense to double up on the cheapest one.

as i read it on wiki it's not entirely correct. even with 500 hp the damage will be calculated before prot spirit. so 80 damage will hit 50 (10% of 500) will make damage and spirit bound will calculate the 80 as superior to 60 and heal for 80.
So you will take 50 damage and be healed for 80 even with 500 or less hp.
