i use multiple builds but the best one for my is:

Don't listen to this fool, his build sucks.
Torment, imo isn't much harder than anywhere else. Therefore, you can just use a general PvE bar - and plenty have been posted around this section.

I always ran heavy mysticism build. Lots of reoccurring burning. Never ran Balthzar as an avatar but I did run Lyssa and Merlandru

In general, Balthazar doesn't do much. Lyssa and Melandru are the best avatars for PvE.

Balthazar gives +33% speed boost and +40 armour. Now, neither of those effects is bad per se, but the thing is that speedboosts don't do much in PvE (most enemies stay in one place in NM), and that +40 armour doesn't seem as nice when you realize that it doesn't stack with the ubiquitous "Watch Yourself!" or with stances like Conviction. It also makes your damage holy, which is nice vs. Undead, but Aura of Holy Might does the same and it's an incredibly good skill.
Melandru on the other hand gives you +100 health and immunity to conditions. 100 health is no small amount, and it certainly ups your survival ability. Conditions are also present in most PvE areas, and AoM means your monk has one less person they need to remove them from.
Lyssa gives you high bonus damage vs foes activating skills. Since caster foes tend to spam like crazy, that means that you get that bonus on at least half your attacks against said caster foes. More damage = win.

Lyssa damages the Heck out of a foe if they are casting spells or anything slow Rez spells Etc..
Melandru – plus 200 HP and conditions do not affect you (awesome)
Take an -1 +15 energy off hand with you just incase you get a neg DP

using lyssa or melandru also means you must use a good strategy when in the torment area too..try not to aggro 2 groups orelse your in trouble.

You forgot to mention that [skill]Eremite's Attack[/skill], [skill]Mystic Sweep[/skill] and [skill]Protector's Strike[/skill] should be stapled to your lyssa bar.

Avatar of Balthazar is a collage of mediocre effects stabled together into one big mediocre elite skill. None of the effects that AoB gives you are worth gibing up your elite for and nearly all of them can be achieved with non-elite skills anyway.
