If you're having trouble with conditions, The build Tyla posted is solid, just add [Assassin's Remedy]. In case you are unfamiliar with how it works, it will remove a condition when you activate an attack skill, so blind will be removed before you attack.
One thing that will help a lot is dump one hero monk (you can always use a henchie monk) for an MM. MM's will provide more targets to soak up damage. Personally, I used an MM with [Blood is Power] so he plays the role of MM + battery.
At least it'll be useful till you can do Sabway, aka triple necro vanquish.

You've got Critical Defenses, Mystic Regeneration and Way of Perfection. The only selfish defense skill you should bring on a 'Sin is Critical Defenses, and even then I won't run it in an 8 man enviroment, Way of Perfection needs you to spec into an extremely weak skill line, shortening the possible attributes, for the spec in Mystic Regeneration you're best off running Healing Breeze. Not to mention you already get an armour buff through Critical Agility as an add-on with the IAS.

Way i do things and is similar to whats been suggested already - any new area i will check out and test with different builds for several runs , each time you die you may learn something - also you can setup a monk with a bonding skill - judges insight ( cant remember if correct ) is a good one that gives the bonded person extra melee dmg. Always with heros have several builds especially the monk heros as ppl will agree ... X build may work one place but will suck somewhere else.
Would also suggest check out the sabway and discordway builds for heros and as your in 6 hero team you can share the necros - 2 good team builds for different circumstances which worked for me thru eotn.
And lastly - never give up ... even the ppl who say xxxx is easy have struggled at some stage and wont admit it ( sorry guys/girls but we all kno its true )

Monk bars are kinda weak with res. Moreso with heroes.
In most of PvE, there will be a res shrine so it'll be better to wipe than to have your Monk Rebirth everyone simply because it's incredibly timewasting, and if you are constantly failing it without DP, or from 15%, you should completely rethink how you're allocating your defenses. Mid-battle res skills would be better for this purpose, and a mid-battle res is a bad idea on a Monk because said Monk will not be doing their job if they are ressing mid-battle. Ressing mid-battle on a Monk would cause more problems than it would solve problems.

I agree that rezzing mid-combat isn't a very good idea.
But I do think it wouldn't hurt to have 1 or 2 rezzes in the group, whether brought by a monk or other party members. Sometimes a party member can get killed during combat. Rezing a dead party member post battle to keep your party full is better than proceeding one person short IMO.
Even if you do bring a rez, just make sure its turned off, and you can activate it post-battle to keep your group closer to full strength

I recommend focussing on killing and taking some skills to negate damage on each char ([enfeebling blood] is the perfect exemple of a good use of one's secondary taht will benefit your team at low cost on your build).
Remember: what's dead does not harm you.

Asuran scan anyone...? That plus a fast atk sin chain is pure ownage...even with weakness on me(my monk was being retarded and not removing) with the scan I hit about 40 or even 50 per hit....Add that to Tyla's build you should be good.

As others have said EotN is 99% condition and hex spam.
Adapt your builds to be a little more removal focused and you should be fine.I just add hex removal to one of my heroes and slot [assassin's remedy] in certain areas or alternately slot [foul feast] + [plague sending] on a Necro which works a treat in EotN.Tyla's monk bars look great although personally I prefer exploiting the spammable heals/removal/mitigation Necros and N/Rits provide thanks to soul reaping.
I also can't imagine any Sin not rolling [splinter weapon] on at least one hero.
Other than that if your H/Hing a lot, and don't already, a little micro by the way of hotkeys goes a long way towards keeping you alive especially in getting your prots and removal targeted correctly.Heroes often prioritize key skills badly and hotkeying a few key hero skills in conjunction with self-targeting can make the game a lot easier, especially if your rolling three caster heroes as I imagine you'll be soaking up a fair bit of the initial aggro and spikes.
Having skills like [protective spirit] and [remove hex] a key press away helps hugely.
[save yourselves!] is a no-brainer if you can get it.
