Alot of very good suggestions have been made and as always not everyone will be pleased. I like the whole Mastersmith idea. As for those that say it's not fair that not everyone will be able to have a perfectly customized weapon available to them. You can. Just like the rest of us so far, go out and find it or buy it. Just as with FoW armor these weapons should be only cosmetically different. Nothing that makes them more powerfull or in any way gives the weilder an advantage over what any other player has a chance of getting in game. The Mastersmith would have to be an end Game Quest.
I would suggest that you only get the quest for him after completing every Mission and bonus, and all quest's/side quests for your profession. Something of a reward for completing every quest/mission in the game.
It was a bit of a let down to finish all the quests/missions (titan quests included) only to get more XP and skill points to add to my collection of over 100sp after unlocking every primary profession and all secondary professions skills for my character.
As for it being for Elite players only I don't think that's going to be a problem. Just as with FOW/UW players will always find a way to make them easier than intended. And after all this is a cooperative play style Game. It's not meant to be soloed. So get some help or Join a guild that will help you get your Master weapon if you want one!

I think this is one of the most creative ideas I've read in a while.
I don't think the quest should be repeatable, but I like the idea of being able to go back, and getting one item forged per visit.

Yeah the dual availability sounds good. If there is a quest for customizable weapons which isn't uber hard to complete, then it would be readily accessible to everyone. Being able to pick out a unique and atractive look for your weapon would be an award for people doing the harder quest, wile the normal quest could grant great stats on a very average looking weapon.
Even though, I still think weapon mod traders should be in the game, if they were, only the skill and damage modifiers would be important on such master smith quests, but it would work either way.
P.S. sorry for being so expressly negative.

For instance, you said fellblade=1shard, shards are dirt cheap. Make it 5 shards or something.
No collector items needed for crafting, just use more of the less common ones.
Make +15% mods more expensive, and different prices go something like this (from most to least)
15% always (UBER EXPENSIVE)
>50
<50
while enched (maybe equal to stance?)
while in stance(maybe equal to ench'ed?)
againsted hexed foe
ETC
make it unrepeatable due to the awesomeness of the weps.

I don't plan on messing up the balance mechanics of Guild Wars.
I'm not sure if you guys have realized. But this quest is available to anyone, its the person's fault if they don't have the party to complete it. If you don't have a good guild or company of friends, obviously you're not playing the game the way its ment to be played; and no matter what you'll be shafted by other things then just this quest with out a strong guild or friends.

Yes, because this shouldnt be cheap. After all you are crafting a weapon unique to your needs and your character, considering theres only going to be one that is like the one you can have (since it has your name), shouldnt it cost more?

That was the part I commented on.
This WOULD be something for the casual player. Last I checked casual players play through the missions and quests and this mastersmith would provide an actual decent reward for those casual players. Weapons with maxed stats and mods with max damage are weapons that are found in high level areas which are also areas found near the end of the game. So the smith should be located exactly there. Near the end of the game or even after beating the last mission. As for the lesser smith that offers maxed, non rare skinned weapons, they already exist. Their weapon crafters and collectors located all over the place. As for rare max stat and rare skinned weapons. They too are available as early as Lion's Arch and even Ascalon as long as you have the money to buy them. The choice is also there so you can have one to match your build. Sure they wouldn't have the unique name but they can have their mods replaced and don't have to be customized so they can be sold later if you get sick of it.
As for the price, I'm sure anet will decide that if they implament this suggestion. However the prices of 'top of the line' weapons are high enough as it is. This is suppose to be something for the casual (average) player and not the repetitive farmer. Also part of the payment is beating the quest.

Oh, you're refering to that. You are correct, about the "standard but useful" items already available through collectors. But, not every item though.
For example:
Weapons with inherent mods such as:
+20% while health is below 50%
+20% while hexed
+15% vs hexed
Those "while in stance", "while enchanted", we can easily acquire from desert collectors. But for the above 3 mentioned, they are yet to be available in the form of collectors. I suspect they might be available to us as collectors in factions, though.
For shields or staves with "special inherent mods", such as:
Shields:
+4-8 Armor vs <insert mob type>
+4-8 Armor vs <damage type>
Shields and Staves:
Quick recovery from <insert condition>
Suffix mod:
Staves:
20% chance +1 to <insert attribute>
We can't craft these yet.
I want to be able to craft these.
Oh, I want to be able to craft "monster mods" for weapon suffix as well.

Weapons:
+20% damage vs <insert mob type>

Go to LA or Ascalon District 1 and put out a WTB. Your sure to get a response. Or how about this, do the quest. It's not like the op suggested this be only available to certain players. All players could get it if they work to it. Get to ascalon, get run to the desert and beacons, get ascended, warp to beacons, get infused then head of the do the last four missions of the game. After that do the quest to get your customized weapon. You could do that in one weekend. So what's the need of a second smith anywhere else in the game? Any quest that will offer the ability to pay for a top end weapon regardless of the skin that weapon can/will have should not be made readily available to anyone the moment they get run to Droks after getting to beacon's. Sounds to me that the op suggested it to be a reward for actually playing the game. God forebid you would actually have to play some of the game to get a top end item instead of farming for it or getting lucky and finding one yourself.
