You won't be able to kick off a jerk if your entire PUG team is full of them, but if there is persistent one or two troublemakers you can get rid of them in 8 party size. Solutions for afkers are good as well.
From my side I would propose the following (if haven't been suggested already, didn't read all pages) - when a person is kicked, team leader has the option to choose a henchmen to replace that person or bring a hero. Henchmen would be chosen from the henchmen list of the town team started the journey, in case there would be a duplicate choice for some reason you are allowed to have two same henchmen in the team. In the case hero is chosen, every team member get's a drop-down list in which he can submit on to 2 of his heroes, of which none were taken to the location, and then once all propositions are in the place players can vote. In case it's 50:50 voting takes place again. In case there is no agreement 3 times in a row, leader choses the hero on his own. The hero which is brought to the team is manageable varying on whose hero it is, and can have changed skillset and attributes for one time. He will use normal items he was equipped with before. Players before submitting a vote will have the option to as well review items and skills of the other heroes. Of course some technical limitations would take place to not abuse it, but for now I can't really think much about possible problems with this system. Just a suggestion.

I don't PuG much, but I have the horrible luck of getting the biggest idiot in the district to join. And most of the time you can't tell until you're in the explorable/mission/dungeon.

It would be nice to be able to remove idiots but it is just an unworkable system. If you say only the leader can call the vote, it requires a majority to kick, and all loot is taken with them, you still have the problem of kicking once no longer needed = waste of that person's time.
Also say you are doing as I do quite often UWSC with a guild team, (not your own guild). 9/10 quests and they say you pay us 5k or you get nothing. They can kick you out easily if you dont all pay. Same would occur with dungeons and so on. Extortion would be ridiculous, (and please dont say everyone would behave fairly because if they did this system would have no point in the first place).

It's a nice idea, and well thought out at that.
Though I believe this could more easily be tacked on to the leeching system in pvp, I have a feeling adding an entirely new system like this would prove too difficult for anet to bother with.
Not to mention that despite the ideas preventing this from being abused, I have a feeling this would cause more harm than good. Being able to kick any given member for poort performance would end up having negative affects on the game's community and leave new players (or players new to certain areas) being kicked by groups that don't have the patience for them.
Perhaps if this system was limited to inactive players (anyone who forced to test the new dishonour system due to their lack of information regarding it will know that the game can easily tell when you are inactive) it would be less open to abuse.
I like the idea of voting, but don't have enough faith in the masses to believe that this would be an adequate safety precaution.
Of course we all know that none of this will ever be added to the game, but it's an interesting concept none the less.

That's a good idea. Instead of creating it, let you report people for griefing, instead of leeching, with the idea that if enough people report them, they can't group with anyone for a specified amount of time, just like a leeching report.
That's a much better solution. You don't end up getting kicked, but you get dishonor points for each person who reports you.

1) If your doing your part in the team and not being a tool then nobody will want you kicked, only time you want someone kicked is when they hurt the team, hence the kick would be EFFECTIVE.
2) Again as long as your doing your part for the team they will not want to drop someone from the team to increase their chances at getting possibly one more gold item
3) Obviously you never bother to READ the actual proposal which stated if you were kicked you would get and omg this is a hard concept, an UNCLAIMED ITEMS WINDOW! I know its hard to believe that this would have to be put into the game, oh wait its already in the game. fancy that. The other less preferred drop recovery is you get a timer allowing you to collect your drops. Finally they cant kick you quick enough for you not to pick up an ecto or w/e
4) I can see this argument actually happening but at the same time it is a weak one. In my book I would see the runner being reported for scamming or a new option called "Boot abuse" which should have a minimum of one week account suspension IF the GM accuses him on scamming someone then forcing the other to boot.

This is a good example but make a boot abuse report that suspends anyone's account for a week, for each infraction. I've been in groups where the monk or necro wouldn't rez unless you paid 500 gold in advance. I reported and e-mail anet with the screens, he got banned for 3 days for his stupid acts...also never had to pay him for the rez he used on me lol.
