Minions stop following when you've got another minion that doesn't or won't follow you. If you summon a minion on a ledge or something where there's no obvious route for them to get to you, not only will that minion not follow you, other minions won't follow. The same happens if a minion gets stuck behind an object, or you get too far away from them, or if they see an enemy that they want to fight, but can't get at. Once you can pick up Taste of Death, I'd recommend bringing it with you, as it will let you kill problem minions. Once whatever minions are causing the problems die, your other minions will start following, so if you raise minions somewhere that they won't be able to get to you, you can just use Taste of Death to kill them off so they won't screw up your other minions.
