Moddok Crevice isn't impossible to H/H in HM. My assassin did it for masters and was easy once I coordinated my heros properly with auto-lock.
Aurora Glade though... I can't see doing that bonus H/H in HM, but if someone has then they deserve a title just for that. Same goes for Eternal Grove.

Dzagonour and jannurs definitly.
There are no words to discribe how much i hate those 2.
A bit easier is Eternal grove.
Tyria -very easy altough I really hate desert missions,especialy the one with corpse trick.

Personally, I'm struggling more with the early game missions... Jennur's Horde and Dragonur Bastion (or whatever it's called) have turned out to be hell, but at least those allow 8-man parties.

I really had no trouble with Nolani Academy, I'm assuming the part which most ppl have trouble with is right after the cutscene where Rurik blows the horn. The key to beating that part is to go ahead of rurik before he starts moving and kill some of the charr in the area. Once Rurik begins to move, and reaches you there really should not be a problem.
I'm just attempting D'Alessio now so I'm glad I've heard already that its a pain in the butt. I'd hate to be sitting at my comp screaming at the screen after having Dorian die for the 6th time

what, bastion and grove were pretty ez
bastion just move all whispers to east then defend center until bosses spawn, from there u take out war boss then run back and defend center, then get monk boss, back to center, etc until u reach ele boss.
grove, get mm healer nuker war/sin that knows how to interupt or deal dmg X2 and mission is done.
i find the hardest of them to be the early ascalon missions :C
as well as the kuunavang one T_T
maybe i need to change my build for those.

I found Elona HM to be quite easy. The only hard ones were the ones that weren't simply 'go to a location and kill everything' like Moddok Crevice and Dzagnour.

Prophecies: D'Allessio because of Dorian as others have said. Nolani is a snap. After the cutscene, use the right path trick to finish the second half of the mission in less than a minute and never worry about leroy Rurik
Factions: Eternal Grove. Just the nature of needing to split the team and finding a good make up is hard. We had problems with it even running ursan so we switch to more tradtional builds and got through it fine.
Nightfall: Jokkonur diggings. Required melee NPC taking 1/4 of an already short slot, plus the speed of that damn skeleton and the environmental effects make is irritating more than hard.

I personally hate many of the old Prophecies Missions. Often Bonus NPCs die before you get there, ffs (Hell's Precipice, Ice Caves of Sorrow, D'Alessio, Divinity Coast...)

Same here, I did Eternal Grove with no problems. I have even helped friends and guildies with it and never had problems.
Hardest I'd say are probably the RoF missions or the ones in the Realm of Torment.

Yep Auora is annoying, in fact I believe that it is the only Proph. mission I could not H/H and was the last one I needed for the title.
Heres what I did, I play ranger, so you may have to adjust. My friend was the monk, he has max drarven title and he brought that running skill that lasts forever, but you need to be drunk.(turn off post processing effect).
Anyways, I brought Sab's 3 necro team with me, my exact build is listed recently in the H/H thread listing Sabs build but what was most important was that I had "finish him," "you move like a dwarf" and an inturpt. This allows you to KD and cripple the runners, and FH spikes the healers which is important because they bunch up and begin to heal each other.
I guarded the middle cap point as long as I could while the runner capped the near and far left shrine. You may have to fall back allowing the enemy to cap but have the runner ready to recap. whoever is not running should control the neco team so they can advance and fall back when needed. they will eventually begin to wear down.
Finally, I did have the grail and pumpkin pie consumable. Also I brought hunny combs to allow my neco team to take virtually no DP (though I believe the MM and SS each only died once.) Hope that helps!

Prophecies: I dont remember the name at this time but it's the one with 15minutes and two monk bosses spamming heals after heals plus the priest (?) that you are supposed to kill is spamming too, to capture it. Not a biggie with curses/interrupts but go there unprepared with mostly physical damage / no interrupt / no anti-healing.
Factions: eternal grove >.>
Nightfall: Rilohn Refuge probably wiped H&H the most, that was back when i didn't have enough interrupt classes (unlocked that is). One Sandstorm and it's all over. For the quickest wipe - award that is. It's not a hard mission on it's own. Eh people ursanway missions dont know the real thing .
Not a mission but still worth to mention:
EoTN: Vloxen Excavations : So ridiculous, if those didn't frustrate you try this one, its a winner of all of these.

FACTION:
Eternal Grove - Very hard to solo but some said they have done it. I've done it with another player and with consumables. Being a Rit, I used spirit spam and MM heros. MM is really a key here.
Actually, its the TIME limits that what makes it hard for most HM Faction missions, not actually the mission itself.
TYRIA:
Aurora Glade - Not hard but tricky is the better word. Hard to solo because of running of portals. But you can do mission and bonus separately which helps a bit.
Actually, a few Tyrian mission I did, I have to do mission and bonus separately. But Im not yet finish with Tyria yet.
ELONIAN:
Haven't faced the devils yet in Elona, only done 3 HM missions in that campaign.

Tyria: Have to choose between Aurora glade and divinity coast. I think divinity coast is more trouble.
Cantha: Gyala hatchery WITH the turtles WITH h/h. End of story. (Did it with consumeables)
Elona: Gate of madness gave me headache.

Tyria: Aurora Glade was my most hated probably, along with the other maguma missions, they all take ages and are extremely boring.
Cantha: Eternal Grove probably, the warrior afflicted boss is really annoying and can kill your party very quickly in combination with his mob. This was the only cantha mission where i actually suffered a party wipe so i would say this is the hardest.
Elona: I did most of Elona with a really good partner+heroes over one weekend so i didn't find it too difficult. I found poghan passage somewhat of a nightmare due to the huge mobs near the end and the tendancy of kormir and the other npc's to die very quickly.
