Almost did this the other day. At the last second I noticed this guy wasn't for Norn points and I quickly hit up wiki to make sure. Lucky too, would have cost me 40000 norn points. I went up 2 Ursan levels.

warning boxes are not a prevention or a fix for lack-of-attention mistakes.
ever deleted something, or not saved something, even though 1 or 2 warning dialogue boxes popped up?
people will make the same mistake as long as it's a possibility to make it, and will complain about it unless there's an un-do option.

No I haven't because I accutally pay attention when I play the game. If the OP or anyone else can't pay attention thats not a reason for Anet to implement a redo or w/e else the OP is suggesting.

Actually, the warning box would only have to say "You are about to hand this book in for <faction> points. Do you wish to continue?"
That's only a modification of the existing message and easy enough to do.
Also, congratulations on always paying attention when you play the game. I'm sure it's nice to be hyper-aware all the time. Some of us aren't, some due to age, some due to drunken playing, some as a result of being tired from working a 60 - 100 hour week.
Your perfection will also tarnish with age and the weight of a real life, have no doubt...

I know how you feel. I did the same thing. Would have been the last book I needed too for any title. then BOOM! Crap, wrong dude...back to the old drawing board. And after a while of the repeating of the same things over and over and over.....it gets so old fast.
I just wish they had bothered to put in enough quests to cover all the points. If not all, at least to level 8. And then maybe some that were only available after reaching level 8.

If you're maxed in any line you've obviously turned in your fair share of hero handbooks before, and know that it takes just a couple of hours to get a new full hm hero handbook.
I wouldn't mind if the npc's stopped accepting hero books when the reputation is maxed, but it seems to me to be a luxury problem, and not something I'd want ANet to spend any effort on. Not when they've got far more pressing matters, like nerfing Ursan, that require their attention.

It is a luxury problem, indeed.
There are studies that people will screw up despite many warning boxes. Sometimes there are so many that people totally ignore them.
Just take more care... and a negative experience is helpful, just imagine getting scammed: Really bad and stupid thing to happen, but you will take more care in future... that is the only positive thing about negative experiences that you cannot totally prevent/avoid.
You still get money for the books, btw. This is why they still take them.

Just show me these studies! I doubt that even if they're "scientific", they're grounded in reality. People DO read warnings, when these warnings are nicely designed and not intrusive (Vista FTL, Apple FTW).
GW is full of these warnings (several "Yes" to click to jump your party to this place?) and this one wouldn't hurt at all. I believe it shouldn't be difficult to implement and wouldn't be intrusive (only happen when you maxed the rank!).

Pretty easy to find an example, people still get scammed despite the text, the warning and the blackened trade window.
Warnings help reduce the amount of scams but cannot eliminate mindlessness. And adding warnings to everything makes people accustomed to them, resulting in even less awareness. People also suggested warnings and check boxes before one can drop items to the ground to prevent people dropping valuable stuff by accident.

If you're actually interested, try Alan Cooper's About Face, Jef Raskin's The Humane Interface, and Don Norman in general.
Human-Computer Interaction (or Computer Human Interaction depending on where you're from) can be eye-opening and exceedingly frustrating.
