The "blurring" is a post-process effect. Your current video card doesn't seem to support it (?)

If you made any change to the graphic card configuration, you should undo it.

When last did you update your gfx cards drivers?
Maybe you should try that, and then turn up all the gfx to max?

The only graphics setting that matters for the drunk effect is post-processing, which your video hardware doesn't support - or so the game seems to "think".
If you claim it worked before and you haven't made any changes to your system, you're probably wrong about at least one of these. I doubt the game just stopped supporting the effects on some particular piece of hardware.
Speaking of hardware, you haven't even told us what video card you have. It could help if you did.

That's a lot of help....
Have you tried turning your graphics to high, and then seeing if you can click it?
And update you graphics card drivers, like someone else said

Are you running -dx8 in your shortcut for Guild Wars? If so, remove it and the checkbox will be reenabled, unless your current graphics card only supports DirectX 8.
