Regrets. Anything that has a 'no turning back' attached to it.

All quests should be repeatable (just with none or less rewards).
There should be a way to go back to pre-Searing (limited, like with the BMP, without inventory, with an assigned weapon, with a fixed set of skills depending on your primary and secondary professions, with a fixed appearance and fixed armor related to level, etc)
There should be a way to retry survivor.
There should be a way to unlock customized items, miniatures and everlasting tonics (destroying them at the same time) and and them being able to craft customized copies of them again for a smaller fee, removing great part of the need to store.
Well... everything else has no regrets.

The only exception I would add to that it's LdoA.
But a way to add it to the HoM without leaving pre-Searing should be added (Probably by going to Melandru's Secret garden and making something there), and once you leave pre-Searing, it should be completely removed from the Character.
LdoA should be a title that exists only in pre-Searing.

Not necessarily a "hate", more of a "wth" moment.

The fact that most of the time (I recall seeing this not happen once), enemy Ritualists who cast item spells (such as VWK) drop them straight after.

i hated how, with heroes and hench, they would start to go in the opposite direction when they neared the edge of the minimap. some kind of autofind inversion...if you flag them to your location it fixes it at least. just a pain in the ass

The only thing I slightly hate are the grind titles.

I've played many other online games, and none have matched guild wars. Atleast title grind is a choice, on the other games you have to grind to level. As for elitism, the last game I played "Conquer Online" has unbelievable inflation on prices. Millionares are about middle class there, lol.
