Naming two kinds of types
One.. Leeches (either unfortunate or a restless passenger without a runner)
This type isn't doing his task due to a wanton desire to destroy the team
Its because
he doesnt have the "veteran" skills, or don't like adapting.
He could be inept, clueless, bad at tactics or just lacks the desire too perform the given task/challenge.
Melee leeches are usually gets away with this behavior, Since a Individual Melee character in general doesn't do by itself a huge impact in the battle. They need to run up & catch individual foes in order to attack.
It shouldn't be the end of the party if one warrior dies. which is why these melee leeches often isn't noticed.
Old prophesies days: farmer wannabe smiting monks, made a bad reputation for smiters.
Old faction days:assassins wannabe tanks, made a bad reputation for assassins.
"worst" :invited proclaimed Healing prayer monk casts Meteor & Meteor shower and complains about energy.
Two.. Avengers or Terrorist
Clueless or Pissed off player or Joker that preys on players. They actually go out to harm the party. Often quite intentional.
aggro a crowd and returns for healing (with aggro) to the back-lining healers.
shadowform assassin runs off compass and returns (during battle) with a huge ball of extra aggro.
Favorit: Not skipping the cut scene?:Necromancer Minion Master rage quits and turns its minions hostile upon the unsuspecting party. Was Common in early factions when cut scenes counted against the overall bonus timer. Also due the cut-scenes degened killed the undead minions .
"douchbag": Condition necro/ranger using Edge of Extinction, Frozen soil, Pestilence, Rotting Flesh and Virulence (elite)
Humanoid foes? Rotting flesh and summons Edge of Extinction.
Several party members dead? Summons Frozen soil. and recast it.

Tell tell signs of a bad PVE party setup
Frequent Dying's and long-winded battles.
Indications of lousy tanks (aggro-holding)
Friendly softies (healers and casters) dying at aggro
Indications of lousy tanks (non aggro-holding)
Enemy healers unmolested
Battles taking to long
Indications of lousy healing
multiple Slow casting spells (1+ s), multiple similar effect spells (3+), party wiping from few non boss foes. Casting damage skills. Needed NPCs dying
Indications of a lousy damage dealer
Battles taking to long
Foes unmolested.
