I have spreadsheets that I use to track what my characters have masters/bonus on, what missions still need to be done, and their birthdays. I also am heavily active in my alliance so I'm constantly having to write things down in a binder for parties, activities, etc (mainly because I'm the kind of person who will forget if they don't write things down). So I'll admit that I do keep records of what I do.

I have a notebook by the computer just for my use, so I can write stuff down when I am on it. Home can be pretty busy, and I don't have too much time on the computer, so I either play a game, or do some homework, never remember where I had left off so write it down.

I usually have scraps of paper and pens around for writing complex stuff down, like the total mats and cost for an armor set, or a certain skill combination I think could be cool but don't have time to test.
Aside from that I write about some of my gaming experiences here, on my blog.

Im very surprised by how much I actually remember after four years of playing this, never really noted down anything. Now if only i can apply this to more useful things in real life . . .

Only really noted weapon storage, as I keep weapons on a mules' heroes if I run out of inventory space.
Was utter hell before I started noting it down, hence the change. xP

I have a stack of scrap paper next to my keyboard, where I take temporary notes. Not only for Guild Wars, I use it for everthing else as well. For GW: how many party/sweet/whatever points still open, timestamp of last Zaishen Elite run (for the 24h point reset), etc. On one permanent (and already very stained) piece of paper there is a handwritten table with the standard buy/sell prices of all materials. I sell if the actual price is above and hold if it is below. And a list of all materials+prices artisans need to create material.
On another piece of paper there is a table with all of the Nightfall chest locations and on what date every character of mine last visited it.
If I go capturing elites for a character, I use a german mapping tool called Visions where you can search for each elite+location and check what you already captured.
I have some mule characters and even a mule account and tried to record each item in text files, but I never keep the record current, so I ceased doing that. I have mules for melee weapons, mules for caster weapons, mules for runes and insignia, etc. I know in what mule I have to look if I need a vigor rune, but I cannot tell if I currently have one without actually logging on with that mule.
I organized my Xunlai chest so I have one page reserved for game books to fill and keep all books there. Not in each character's inventory.
At memorable events I take screenshots.
For information and reference I have several bookmarks in my browser to important articles in guild wars wiki, in the update notes, or to important articles or forum threads in several gw community sites and forums.
I made an Excel sheet for planning wisdom/lucky/unlucky/treasure hunter titles, so I could see in what area I should make chest runs to max out every title at the same time and how many runs were left.

I think my Excel file is up to about 10Mb now. There are some apps that perform similar things but I like to have my own style and layout.

I have a notebook by the computer where I make temporary notes about things I need to remember in the short term. It's nothing very organized, and probably nothing that would be comprehensible to anyone else reading it.
I've been kicking around the idea of doing an IC blog for my character to ponder her experiences. I haven't gotten around to it yet, but I'd like to give it a try at some point. (I doubt anyone but myself and a few close guildies would read it, but it's mainly for my own amusement anyway, so that's totally fine.)

I've never taken notes for Guild Wars.
Though, I do take notes when I play a game that has puzzles that took effort to solve. I write down the solution so next time I go through I don't need to resolve it.

The one thing that I keep track of is all my armor dye combos.. Just incase I need to reference back and see what I used.
I sometimes write down what materials are needed for some items (consumables) and maybe where some of the crafters are located. ^^
I also had a excel of my treasure chests that I used to run whenever they reset.. but.. i've gotten lazy and haven't messed with any of those chests in forever.

I have a little spiral-bound notebook I keep track of some stuff. Armor I want, tome prices, etc etc etc. Nothing special, just for quick stuff to keep track of so I don't have to hunt through Guru. :P

Used to use Guild Wars wiki to keep track of stuff but haven't updated that since last time I logged in. So now I just keep everything in my head where it belongs. o.o

Just scimmed over this quickly. In short, if something interesting happens or something's worth talking about, it goes on my blog (http://mikeofwffguild.blogspot.com)
My desktop's littered with random notepad files of things I intend to do too.

Oldstyle pen(cile) and paper for me
I had a list with Pre quests for my LDoA character. One day my partner decided the desk is too messy and needs some clearing up... I came back from work
- Hunny where is that piece of paper that was near the keyboard?
- Oh I thought you don't need it...
Since then she doesn't touch any papers. Pens are going missing though ;D
I remember having a notepad back in the days when build templates weren't available. Going from healing to prot to 55... heh, /sentimental
