Just days before treasure hunter and wisdom were made account wide titles, I deleted my warrior with rank 4 in both.

I was in between MQSC runs and was in a hurry to buy lockpicks but ended up buying 50+ vials of Gray dye.

Tried to sell a tormented shield few days ago. I forgot I put it in my HOM so it was customized. Needless to say I pissed someone off real good. I apologized but I can understand if he thought he was being scammed

Uhm, well, lets see...Oh Yeah! Came back from a 3 year break, over-dyed my OLD black warrior's armor with new white dye, not realizing the black dye I had was completely irreplaceable and would never see it again. ugh.

Yep, i'm around 50 minutes away now, getting Ale-Hound is by far the biggest waste of my life I have encountered so far.

Letting my 7 year old daughter have her own account and join my guild. I walk in on her spamming the crap out of alliance chat and Kamadan local chat. She also decided she didn't like her ranger and deleted him (along with all the runes, weapons, money, mini pets etc....).

I so feel you there. I was pretty close to giving up on the game due to that badly designed mission too. I wonder how many thousands of potentially loyal GW users never came back due to suicidal Rurik.

Lol I guess I'm lucky that I never even knew that there was a mission there till I was on the fire islands already...

The first character I'd created (just after GW came out) was a male wammo and have worked him up to level 8 before going post-searing. Never knew you had to do missions (in post-searing) nor about clicking on the "Enter Mission" button when you entered certain areas. Basically I just thought all you do is take NPC quests, level up, earn gold, get better armour, make your way to the next area, etc. (Old school RPG :-P) Then one day I saw the "Enter Mission" button while in Fort Ranik decided to click on it and was teleported into the mission...alone. Needless to say my toon was slaughtered within 5 mins. lol.

Never really found a problem doing this mission in NM. However, I have found HM to be next to possible to do on this mission until I read the tip/note from one of the wikis that uses the game mechanics to your advantage...
Basically foes do not spawn in-game unless a real player is near radar range. So after the suicidal Rurik's cut-scene at the wall instead of following his route you head in the other direction (eastward) and make your way to the end-boss Bonfaaz Burntfur as quickly as you can. With luck you should be able to kill him before Rurik triggers the second cut-scene.

Overestimated the competency of the support team.
Cost me 9,900~ hours and way too many irreplaceable items.

My first couple times through Nolani in NM on new characters, I think the friend who was teaching me the game led me around that way. My first time back to it by myself, with just heroes, was a few months later for the Zaishen quest, and I decided to do it in HM. I couldn't really remember which way to go, so I just followed Rurik. The heroes were slaughtered (repeatedly; I think I was carrying a hard res on my ranger's bar back then), but by some miracle Rurik and I made it to the end.
I later told my friend about the nightmarish experience, and he said, "Why didn't you just go around the other side?" D'oh!

Back when I was a noob and platinum was showed as a logo and not as 000 [so 5k looked like 5[logo] instead of 5,000 gold]
I had a worthy axe from TOPK [barrageteam] 36k. Someone showed 36gold and I accepted because if it looked like 36plat.
Heartache
