I used spirit spam with no problems. Just can't take on more than one, maybe two mobs at once.

Used the R/RT spirit farm build to get them. It was a little tough when it got to the warrior boss but it's manageable.

Only problem comes from when there's an obstruction and the spirits can't target the monk who's healing from behind it.

They drop everywhere, but they are a very rare drop. Supposedly, they drop from foes who can drop crafting materials. There may be a level requirement as well.

Based on what? I remember hearing something like that a while back, but that was before the Nik stuff was implimented. Also, it's possible that they adapted the code, thus making it different and all the little code would be unique.
That aside there's another way to tell, if people are willing to participate. Simply figure out how many hours you've played since you've last received the gifts. In most cases for me the only play time I get as of late is farming the items, so it's about an hour a week. I would think that if people have logged more hours they've got a better chance at golds, greens and minis then those that log only an hour or two a week.
As an answer to the sapphire question, yeah, pretty much anything can drop sapphires. Way, way back, I got a pair of sapphires when Green farming in Eternal Grove explorable (ie when Scar Eater was still worth 20k), from a pair of wardens in two seperate runs.

If so the level req can't be very high as i had Sapphires drop from the lvl 11 Fire Imps outside LA before.

I think what happened is when they were first added to the game, they dropped from such a seemingly random selection of enemies that a few people on GuildWiki decided it was valid to assume they dropped from everything, and that became the conventional wisdom that everyone quoted. Even I went along with it until I saw Linsey's comment there, then I looked through the history of our sapphire/ruby articles, and realized that it really was only the same small group of monsters that people had been adding repeatedly.
