Try re-installing your Graphics Card driver, also, try the 'latest' version 81.98 from www.nvidia.com .

That sounds like your video card drivers.
I would get them upgraded here: http://www.nvidia.com/content/drivers/drivers.asp
Make sure disable any antivirus software before installing the drivers.

It is his video card drivers as I ran a scan and NV4-related files are in my drivers folder and some were titled nv4_disp.dll so I presume disp for display, so it's why I said try reinstall your Graphics Card drivers.

before the last update i was doing fine with my 6600gt @81.89 - but now i am getting assertion errors on the FrApi.cpp aka client crashes to desktop.
70 degree celsius can be a bit much - try and get rid of all dust that is cluttering in your gpu fan. that can decrease the heat by a few degrees.
do these square textures go away after a while or a quick minimize and restore?

To get rid of the squares change to lower detail. I had those on my ti4200 after my 6800gt died and played on max detail, but changing it to lowest solved my problem.
Btw i read that the 71.xx drivers are good stable ones, so i am not sure in that switching always to latest drivers are the best solution.
