what version of VISTA did you have the misfortune to so called upgrade to?
dx10 is vista only and that needs an 8 series card not a very low 5 series
vista is not recommended by gaming mags for playing games reporting about a 25 pr cent performance hit and incompatability issues

Vista runs Guild Wars fine I've been playing it in Vista for months with no problems. Try deleteing your Guild Wars directory and reinstall you could have a corrupt gw.dat file. Might want to check other games too if you have problems with those try updating or reinstalling video drivers, sound drivers and directx10.

Basically, your system is kinda old. That's probably why you have problems with Vista. If i were you, i'd try reverting to xp to see if there's any difference.

While your system technically meets the requirements to run Vista, in practice you need much more than that to run Vista and get any playability out of games. Hell, Vista itself could easily be eating up over half of your system's RAM. DirectX 10 (or 9 for that matter) doesn't even exist for you. A Geforce 5200 is only a DX8 card. Even the things that could be construed as "advantages" of Vista are unusable to your system.
While there's a lot of possible issues that could cause the problem you're experiencing, if it were me I'd go back to XP before I tried anything else and see if that helps.

Before you bought your system you should have checked the requirements for Windows Vista, on the microsoft webpage. It checks your specs and tells you if you have a good enough set up for Windows Vista. You don't have nearly a good enough setup for Windows Vista, so you running directx10 is a really bad idea. Try downgrading to Windows XP Home Edition Sp2 (Which is the best for your setup). You're Graphics card and RAM are not good enough for Windows Vista (it is a bad hog for a lot of computers).
So downgrade to XP and reinstall all of your stuff. Or just buy a Gforce card that supports your motherboard. The highest PCI 2.0 or PCI Express x16 Gforce card is a 8800 Ultra GTX (Takes alot of money...try an 8800GT for $250..or go cheaper). The highest Gforce AGP card is a 7800 which is fairly cheap. Also upgrade your RAM based on your motherboard (either 240 or 184 pin, most are 240 pin but make sure by looking up your motherboard or calling your manufacture (if its not custom built).
Check all this out and see what happens.

It's actually the worst DX9 card ever released next to the ATI Radeon 9200.
It supports DX9 and all, but very very poorly.
Nevertheless, if you're using Vista with DirectX 10, chances are it's just too much for your computer's processor or ram to handle, there's also the chance that you need to install DirectX 9 (even though you have DirectX 10 already with Vista)
that can be acquired over at microsoft.com, but really, GW should be able to run without it anyways...
other recommendations are to just run windows update, visit nvidia and install the latest drivers available, or even get older drivers if the new ones are causing the problem in the first place...
PS
I've also been suggested by Anet in the past to apply this method to the game to make it run on lower versions of DirectX (8) to get it to run without problems or issues, maybe it will help you:

"C:\Program Files\Guild Wars\Gw.exe"
Modify the line in the run window to read
"C:\Program Files\Guild Wars\Gw.exe" -noshaders -dx8
4. Press OK to run the game.
Please try that and let us know what happens. .
