I know they drop in the UW, but what about the inscribable ones?

I've been told they drop in HoH, but where else can you get them, what mobs or what chests in what map??

after about 50keys spents in the realm of torment, i havn't seen one drop. do they drop in one particular realm. i did check up on wiki before hand.
