The healer shouldn't be the named caller, but he should be the only one apart from the named caller allowed to call. If the healer starts taking the heat, then the fastest way to fix that is C followed by ctrl + space. As the healer doesn't normally call, everyone knows that it's extra urgent when he does.

As for who should normally call, I would either vote a Ranger to pull mobs in, or a Warrior tank to draw aggro.

CTRL SHIFT SPACE - calls a target without initiating combat (I'm targeting XXXX). There are times when it's useful to call a target without initiating action against it.

And a tip with henchmen, if you do ctrl-shift-space to call a target that's near your visual range circle on the compass, the hench will run to and attack that target. It's useful if you're a caster or worse a healer, since you don't have to be the one running out there initiating aggro to get the hench to attack.

I also find that henchmen will respond pretty quickly with regard to changing targets if you call each change.

I'll chime in as another healer saying that healers shouldn't be primary callers. I'm too busy managing the damage to pay attention to what's being fought generally, though it seems that somehow I'm the only one that ever notices the enemy healers in PVE.

There are exceptions to the rule of focusing fire, but I don't think the people qualifying for the exception have a problem understanding that. I think it's more just a matter of people being lazy due to the easiness of the early game. Early you can charge in and sweep through and kill things pretty easily. Death is rare. Mid-game people start having problems but can usually brute-force though. That's where I had problems with groups calling me annoying for tying to get them to work together. The response was that they were doing okay. Now I'm out in the desert, 1/3 of the way through ascension missions, and having a hell of a time trying to keep alive groups with people that still haven't learned their lesson.

It's not hard being designated caller. I do it all the time with henchmen and usually have an easier time since they obey (if only I could make them pull). I think people either don't see the value or are too lazy (either to call or to bother to see the value).
