Default BUG: Boss's Appearing Twice
Ok during one of my chest runs I found two bosses that appeared more than once, I didn't get the screenshot of second but I did however get one of the first.

2 mesmers bosses spawn in that location: Xisni Dream Haunt and Deeproot Sorrow
are you sure the second one was the same as the first?

Never noticed this one, but I did see (no screenshot) two Kenshi Steelhands apear when I did the "Eliminate the Jade Brotherhood" quest. One was at the quest spot and the other was in his usual place, they were farily close together.

nvm the OP is right, I just capped a skill there and the boss had a twin brother:
