I don't know if this is a similar situation but as of the latest patch I can't either. Shortly before the patch the game froze twice in windowed mode, blacked out and then returned to normal after a few seconds.
I commented on it to my guildies and some mentioned having similar experiences recently.
After patching, when I try to load GW I get :"Guild Wars was unable to initialize 3D output for your system."
I have to admit that I am not very PC literate and don't know what that means...
I have DX 9 and apparently no new driver updates required.I use a Fujitsu Siemens T7200 2 GHz 1 GB RAM Intel Core 2 Duo with ATI Radeon Mobility X1800

Any suggestions appreciated.

Gah, I've sent alot of reports of issues like this to Anet too, and they always say it's on OUR END and it's PISSING ME OFF! Because when so many members of the community are reporting this. I did a Network checker, contacted some peoples, ectera. My cousin is having this insane lag lately, as are plenty of people around. My ping is 90 in other MMORPG's and FPS's, so I have no idea where they get the idea it's my fault. I've given them different log files a bunch of times, and then they ask me what my connection type is...when they already asked me to, and I did send them, a Network log that contained that all, obviously showing they don't read them. Anet may listen to the game for Gameplay matters, but Software matters, they stay away from.

Nv4_disp.dll is one of the nvidia driver files. Try uninstall the old driver from the control panel first, reboot then install a new fresh driver from nvidia and see if the error still come up.

for the code058 problem

reistalling gw should help.
in the future make a copy of gw.dat file instead of reinstalling

I have the newest drivers already installed, i also uninstalled them , used driver cleaner and reinstall still no luck i have 2 systems identical they are both doing exactly the same thing.

Im a computer tech with over 20 years did a whole checkup on both machines no problems on my end.

Funny thing is day before patch i was playing with no problems for hours on end.

Ok i loaded older drivers in a few revisions back i can play again lol something they changed made game incompatible with newer drivers?.

Update to the latest drivers? You must not know about Nvidia drivers, the 90. series is still better than the current. I update past them and I loose 40 FPS, which is unacceptable. :P

It is possible files in the gw.dat got corrupted during the update.

Rename gw.dat, run guildwars, it will download just enough to launch, login / see if it crashes. If it does, Gw needs re-installed.

(edit) just saw your edit - I'd report that to anet about that driver version + card = crash.
Won't be the 1st time that's happened.
